Family Carers offers a specialist re-ablement service. A range of services to adults with different support needs are provided, to enable NHS hospital beds to be available for acute patients by providing 24-hour live-in care to many elderly and vulnerable people in their own homes.
The aim being to minimise your stay in hospital, Re-ablement provides a time-limited, outcome focussed intervention.
Designed to help people accommodate their illness by learning or re-learning the skills necessary for daily living:
- helping people ‘to do ‘rather than ‘doing to or for ‘people.
- outcome focused with defined maximum duration.
The Objectives are:
- to maximise users long-term independence, choice, and quality of life
- to appropriately minimise ongoing support required and, thereby, minimise the whole life-cost of care.
Examples of some of the elements
- personal care.
- cooking, preparing meals and helping to eat.
- building confidence
- shopping, pension collection, laundry, and other household tasks
- coping with poor memory
- social and leisure activities
